Care & Support Worker - Reablement (26/27)

Salary: £13.45 - £22.50 per hour
Location: Cheshire

 

Connect2Halton is recruiting Care & Support Workers - Reablement to work with Halton Borough Council, supporting residents in their own homes and helping them regain their independence.

As a Reablement Care & Support Worker, you will provide person-centred care and practical support to residents within the community. You will work with individuals to help them develop or regain the skills and confidence they need to live as independently as possible.

This is a rewarding role where you will make a genuine difference to people's day-to-day lives.

Key Responsibilities

  • Providing compassionate, person-centred care and support to residents in their own homes.
  • Supporting individuals with their daily routines and personal care where required.
  • Encouraging and supporting residents to regain independence and confidence.
  • Helping residents develop practical skills and maintain their ability to live independently.
  • Supporting with tasks such as preparing meals, shopping and maintaining a safe home environment.
  • Monitoring residents' wellbeing and reporting any concerns appropriately.
  • Working alongside colleagues, families and other professionals to provide effective support.
  • Maintaining accurate and up-to-date records in line with Council procedures.
  • Promoting dignity, choice, privacy and independence at all times.
  • Travelling between residents' homes across the Halton area.

About You

We are looking for caring, reliable and motivated individuals who genuinely want to make a difference to people's lives.

You will ideally have experience working in a care or support environment, although a positive attitude, willingness to learn and a person-centred approach are equally important.

You should be:

  • Caring, compassionate and patient.
  • Reliable and able to work independently.
  • Confident communicating with residents, families and colleagues.
  • Respectful of people's individual needs, choices and dignity.
  • Comfortable working in residents' homes and within the community.
  • Able to manage your time effectively and travel between appointments.
  • A driver with access to your own vehicle - this is essential for the role.
